Foreign pensioners who relocate to southern Italian municipalities can benefit from a 7% flat tax regime under Article 24-ter of the Italian Income Tax Code. This preferential tax treatment applies to all foreign-sourced pension income for up to ten years, provided retirees establish tax residence in eligible municipalities with populations under 20,000 inhabitants.

Eligible regions include Sicily, Calabria, Sardinia, Campania, Basilicata, Abruzzo, Molise, and Puglia. Applicants must not have been Italian tax residents for at least five years prior to application and must relocate from countries with tax cooperation agreements with Italy.

Successful applicants receive an initial residence permit valid for one year, renewable annually upon demonstrating continued compliance with income requirements and physical presence. Retirees must reside in Italy for at least 183 days per year to maintain their residency status.

The visa grants access to 26 Schengen countries for visa-free travel and allows holders to register with Italy's public healthcare system for approximately EUR 2K annually after initial private insurance coverage.

After five years of continuous legal residence, elective residents become eligible to apply for permanent residency, subject to Italian language proficiency testing and other integration requirements. Italian citizenship becomes available after ten years of legal residence, requiring a minimum of 270 days per year physical presence throughout the qualifying period. Italy permits dual citizenship, allowing retirees to retain their original nationality upon naturalization.

The application process typically requires two to four months for processing, though Italian law stipulates a maximum 90-day decision period. Applicants must demonstrate suitable accommodation through property ownership or a minimum one-year lease agreement.
